| The normal cycle of hair growth lasts for 2 to 3 years and hair grows approximately 1 centimeter per month during this phase. About 90 percent of the hair on your scalp are growing at any one time. After 3 to 4 months, the resting hair falls out and new hair starts to grow in its place. It is normal to shed some hair each day as part of this cycle. However, some people may experience more than normal hair loss.
Hair loss of this type can affect men, women and children. In men there may be many causes of extreme hair loss. These include
medications, surgery, biochemical and haematological abnormalities, fungal infections, stressful life events, hormonal imbalances conditions such as Lupus or Diabetes. 95% of male patients affected by extreme hair loss fall into a category called Androgenetic Alopecia, more commonly referred to as Male Pattern Baldness.
By the age of 35 it is estimated that two thirds of men will experience a degree of appreciable hair loss and by the age of 50 approximately 85% of men have significantly thinning hair. It is estimated that it takes up to 25% of total hair loss before a patient actually notice visible symptoms.
Around 25% of our patients experience hair loss before the age of 21. Unfortunately, hair loss impacts upon many facets of a man's life and his psychological well being. Our patients often report lower levels of self esteem and problems in their personal relationships. Androgenic Alopecia Androgenic Alopecia or Male Pattern Baldness (MPB) is responsible for hair loss in the vast majority of cases. | In men, the primary sex hormone responsible for secondary sex characteristics is called Testosterone. This is broken down to another form of the hormone called Dihydrotestosterone (DHT) by an enzyme called type II 5 alpha reductase. DHT is responsible for Male Pattern Baldness in genetically susceptible men.

When we mention genetically susceptible men, what male pattern baldness sufferers are actually inheriting are hair follicles with a genetic sensitivity to Dihydrotestosterone (DHT). Hair follicles sensitive to DHT begin to miniaturize, shortening the lifespan of each hair follicle affected. Eventually these affected follicles stop producing cosmetically acceptable hair. Male Pattern Baldness can be characterised by a wide array of symptoms. For some patients, this may start as diffused thinning over the crown, in others a receding hairline, particularly the temples and crown. Hair in the temples and mid-anterior scalp areas appear to be the most sensitive to DHT.

| Prescription medication is offered to our patients only after a specialist Doctor’s consultation, history taking, blood tests if appropriate and counselling regarding the medication. There are a limited number of prescribed treatments available on the market with solid scientifically proven results. The main goal of our therapy is to reduce the concentration of DHT (dihydrotestosterone) in the scalp and blood serum. Our specialists use medications that inhibit type II 5-alpha-reductace, the enzyme that converts testosterone into this more potent androgen DHT (Dihydrotestosterone). Studies have shown the DHT plays an important role in shrinking hair follicles and hence hair loss. American research in the long term Propecia Study proved that hair loss can be abated and actual re-growth can be initiated with the right prescribed treatments. Medications such as Propecia can effectively lower DHT levels by as much as 60% when taken daily, bearing in mind that it is the DHT that shrinks or miniaturizes the hair follicle, which eventually leads to baldness. This 60% reduction in DHT has been clinically proven to stop the progression of hair loss in 86% of men taking it the drug during clinical trials. Other studies of medications that yielded higher suppression levels of DHT in the scalp and serum included the medication Dutasteride manufactured by GlaxoSmithKline, which provided up to 95% suppression of DHT. Initial US phase II trials in 2002 did reveal superior clinical outcomes compared to Finasteride alone at 12 and 24 weeks. A 6 month Phase III Korean study was commenced by GlaxoSmithKline in 2006 investigating a daily dose of Dutasteride for Male Pattern Baldness. These trials were successfully completed in 2009.

Regrowth is more pronounced at the vertex than in the frontal areas and is not noted for at least 4 months. Patients who respond best to this drug are those who have a recent onset of Androgenetic Alopecia and small areas of hair loss. The drug is marketed as a 2% or a 5% solution, with the 5% solution being somewhat more effective. A 48-week study compared the 2 strengths in men. Findings indicated that 45% more re-growth occurred with the 5% compared with the 2% solution. In general, women respond better to topical Minoxidil than men.
